Who Can Enter
To be eligible to participate in this giveaway, participants must be U.S. resident, are 18 years of age or older at time of entry and reside in one of the states where body + face Massage Heights Retreats currently operates. While this contest is open to only legal U.S. residents, Massage Heights only has body + face locations in 16 states (which may be subject to change). Employees of Massage Heights (“Sponsor”), and any of the Sponsor’s parent, subsidiary, or affiliated companies, affiliates, or any of their advertising and promotion agencies, or any other entities involved in the administration of this Contest, as well as all such employees’ immediate family members of each are ineligible. The term “immediate families” includes spouses, grandparents, parents, siblings, children and grandchildren. All federal, state and local laws and regulations apply. Void where prohibited by law.
How to Enter
No purchase is necessary to enter. A purchase will not improve one’s chances of winning. Only one (1) entry per person.
To enter, visit any Massage Heights body + face Retreat location between Wednesday, October 1, 2014 through Friday, October 31, 2014 between the hours of operation to receive a complimentary “Face Map” analysis and to fill out an entry form with your name, phone number, address and age. Each contest participant will also be required to sign a disclosure, acknowledging that they have read and understand these contest rules.
The contest starts on Wednesday, October 1, 2014 at 9:00am and all entries must be received by 9pm on Friday, October 31, 2014. Sponsor is not responsible for lost, late, or misdirected entries, printing errors, server unavailability, computer or any other electronic malfunction.
On Monday, November 10, 2014 one (1) winner will be selected by random drawing from among valid entries.
All entries received that are mutilated, tampered with, illegible, or from ineligible entrants will be void. All entries become the property of Sponsor and will not be acknowledged or returned.
The Prize(s)
One (1) winner will receive a one (1) year Massage Heights Heightened Membership and one (1) year supply of Dermalogica products which includes a cleanser, exfoliant, toner and moisturizer based on the face mapping of the winner, valued at $1,719.88. If the winner is an existing Massage Heights member, the one year membership will be activated the date the current membership expires; therefore, prizes may not run concurrently.
Winner will be announced on Facebook and formally notified via email. Winner must sign and return a release and affidavit of eligibility, which must be received within 10-days of notification, at the time winners claim their prize in person at the Retreat. Winners must show picture identification to claim prizes. Sponsor reserve the right to examine additional identification and may choose to accept or deny awarding the prize based on the identification presented.
If a winner cannot be notified within 10-days or does not timely return the affidavit of eligibility, the prize will be forfeited, and an alternate winner will be randomly selected from among remaining valid entries.
There will be no substitutions or cash alternatives for prizes except at the sole discretion of Sponsor. The Sponsor has the right to substitute a prize of similar value. No transfer or assignment of prizes is allowed.
The winners are responsible for paying all local, county, state and federal taxes on prizes based on the estimated retail value of the prizes as set forth in these rules.
Only one (1) prize will be awarded per person.
Sponsor and any of the Sponsor’s parent, subsidiary or affiliated companies are not liable if the provider of any part of the prize fails to satisfy its obligations to furnish its portion of the prize.
In claiming your prize, you acknowledge that Sponsor and any of the Sponsor’s parent, subsidiary or affiliated companies have the right to publicize your name, character, likeness, photograph, voice and the fact that you are the winner for promotional purposes without financial remuneration.
In claiming your prize, you acknowledge that each Massage Heights Retreat is independently owned and operated.
Odds of Winning/Miscellaneous
Odds of winning depend on the number of entries received.
Complete contest rules are available Monday through Friday during normal business hours at Massage Heights, 13750 US Highway 281 North, Suite 230; San Antonio, TX 78232 and on Massage Heights blog at massageheights.com/blog.
For the name of prize winner send a separate, self-addressed, stamped envelope to Massage Heights, 13750 US Highway 281 North, Suite 230; San Antonio, TX 78232 within 20 days from the drawing date, and indicate which drawing/prize date.
This contest is subject to all applicable laws and regulations and is void where prohibited.
Sponsor and any of the Sponsor’s parent, subsidiary or affiliated companies accept no responsibility or liability in connection with any injuries, losses or damages of any kind caused by or resulting from the acceptance, possession or use of any prize awarded hereunder.
Sponsor cannot be held liable for any lost or stolen entries, prizes or contest information.
Sponsor reserve the right to make rules and contest changes and change the aforementioned contest dates.
By entering this contest, each entrant waives any and all claims of liability against the Massage Heights, any sponsors, Facebook.com, and their respective parent, affiliates, officers, directors, employees and agents, for any and all claims, loss, damage or injury which may occur as a result of entrant’s participation in this contest.
